With patch 9.1, Blizzard wanted to give Mythic Plus players in WoW a similar incentive as PvP players. For the best of the best, in addition to various rewards along the way, there should be a special title waiting at the end for them to show off a bit and flaunt their successes. So for the current season, the title "Tormented Hero" was introduced - the counterpart to the PvP title "Gladiator". However, the developers didn't go into much detail with the introduction, which is why a bunch of questions about the new title arose in the forum and the developers now felt compelled to clarify a few things.
The developers clarified once again that only the best 0.1 percent of all characters will receive this title. How many players that will ultimately be depends on the amount of characters, of course.
In addition, a distinction will be made according to faction and region. So there will be several rankings, all based on the respective official mythic score. As a result, the competition in some areas might be lower than elsewhere. For example, as some players correctly pointed out, the Alliance has fewer high-ranking players but about the same number of characters, which would make it easier to secure the title.
01:31To counteract this a bit, no characters that change factions on or after January 18, 2022 will be considered. There would also be a one-week period after the end of the Season where the developers would take a close look at the leaderboard and remove any cheaters or such.

A few questions have come up about the Tormented Hero title that was added for Season 2, and we'd like to take the time to answer those questions here.
This new title will be awarded at the end of the season based on the Mythic+ score to the top 0.1% of player characters from both factions. It is designedas a counterpart to the most prestigious title in PvP, where a small number of players earn the title of "Gladiator" each season.
Season 2 will end with the maintenance period that includes the End of Eternity patch. At that time*, the leaderboards will be frozen and a one-week post-season will begin. After we verify the results and remove cheaters, the top-scoring 0.1% of each faction will receive the title of Tormented Hero.
Since each Mythic+ region has a different number of participants per faction, we expect that the score needed to place in the top 0.1% will vary significantly from faction to faction. To make the faction rankings as fair as possible, we are setting a cutoff date for faction changes. Any player character who changes factions on or after January 18, 2022 will not be eligible for the title of Tormented Hero regardless of Mythic+ rating.
We have also received questions about the current rankings. More details on the deadline will follow as the end of the season approaches. Until then, you can use the Mythic+ tracker from Raider.io here as a guide.
